Care Plan Page Overview

Use the Care Plan to track Client needs, related memos, assigned resources, and follow-up actions. Organize key client information through linked memos and assessments, providing a centralized view for managing Client care.


What You’ll Find on the Care Plan Page

  1. Problem Needs:

    • Notes-Only Needs: These track issues without assigned resources, using memos for ongoing documentation.
    • Needs with Resources: These have assigned services, tracked through related actions and follow-ups.
  2. Memos:

    • Assessment Memos: Automatically linked from Assessments when Care Plan-enabled.
    • Action Memos: Created from follow-up actions related to problem needs.
    • Follow-Up Memos: Document completed problem resolutions.
  3. Activity Details:

    • Activity Type: Describes the source or reason for the memo.
    • Activity Date: Date the activity or action occurred.
    • Date Added: When the activity or memo was recorded in the Care Plan.

How to Access the Care Plan Page

  1. Navigate to the Client Profile

  2. Click Care Plan
    Note:The Care Plan opens in a new browser tab.


  1. Click Expand All to view details of all problem needs or click on specific problem needs or memos to view the details.




Viewing Care Plan Memos

  1. Linked Memos:

    • Memos added through Assessments are automatically linked to the Care Plan when the Assessment is Care Plan-enabled.
  2. Accessing Memos:

    • Select a memo from the Care Plan list to view details, including the Activity Type and Activity Date.

Adding Memos to the Care Plan

  1. Create a Memo:

    • Add a new Memo through the Client Profile under the relevant Care Plan entry.
    • Ensure the selected Activity Type is Care Plan-enabled.
  2. Linking the Memo:

    • If the Memo Activity Type is Care Plan-enabled, it will link automatically.
    • Use the New Memo button to enter additional details as needed.

Managing Problem Needs 

  1. Problem Needs Without Resources:

    • These are labeled as Notes Only needs and are tracked using memos without assigned resources.
    • Updates are added through memos for continuous tracking.
  2. Problem Needs with Resources:
    There is a nested hierarchy of data capture in the Care Plan:

    • Resources are assigned to Problem Needs
    • Actions are assigned to Resources
    • Follow-ups are assigned to Actions

       

  • Use the plus icon to add a resource, action, follow-up, or memo to a problem need
  • Use the edit icon to edit an entry
  • Use the delete icon to delete an entry



Completing Problem Needs

Update the Problem Status:

  • Select New Action and enter follow-up details.
  • Update the Status to Goal Met if the problem need has been resolved.

Important Notes

  • Care Plan-Enabled Records Only: Only Care Plan-enabled memos and Assessments are available for linking. If an Activity Type is not enabled, it cannot be added to the Care Plan.
  • Activity Date vs. Date Added: The Activity Date shows when the memo or action occurred, while Date Added shows when it was recorded in the Care Plan.
